a) the usual name of the player (last name and first names) as it appears on a passport, BC Driving License and the game card, your home address and e-mail contact.
b) If requested, a passport style coloured digital photo as a JPEG file;
c) the player's FHBC registration number. If you are a new player to the league FHBC will assign you a new registration number, and
d) payment of the registration fee accompanied by signed waiver forms.

Important note for team contacts: block registering for a team or club is discouraged because participants must supply their individual information and sign their own waiver forms, otherwise the insurance coverage of the player may be void.

The fees charged by FHBC go to support hockey in our province and our national organization, Field Hockey Canada.

You or your team captain must display photo ID to game officials when requested. Failure to produce a current ID will make you ineligble to play. Anyone playing without a current ID will earn a team penalty of the loss of points earned in the game in which the ineligble player participates.

The additional requirement: team captains are responsible for ensuring that all your team's registered players have read and agreed to the Code of Conduct (VMFHL CODE OF CONDUCT - see above).
